چکیده

An electrocardiographic recording obtained during diagnostic evaluation of recurrent transient loss of consciousness in a 53-year-old man is presented. Carotid sinus massage (CSM), having elicited a ventricular asystole of >5s duration was deemed to have provided a possible diagnostic basis for syncope. However, apart from the pause and somewhat unexpectedly, CSM also suppressed preexisting frequent ventricular ectopy. Explanations for this unexpected finding can only be considered speculative, but include direct CSM-induced parasympathetic suppression of ectopic activity at intra-ventricular pacemaker sites, concomitant diminution of sympathetic neural activity at ectopic sites, or interruption of 'linking' of normal ventricular activation to initiation of premature ventricular contractions.

Amnesia for loss of consciousness in carotid sinus syndrome: implications for presentation with falls.

OBJECTIVES The goal of this study was to compare the clinical characteristics of patients with carotid sinus syndrome who presented with falls with those who presented with syncope. BACKGROUND Carotid sinus syndrome presents with both falls and syncope. The reasons for this differential presentation are unknown, but amnesia for loss of consciousness may be the underlying cause. METHODS Two ...

Evaluation of syncope.

Though relatively common, syncope is a complex presenting symptom defined by a transient loss of consciousness, usually accompanied by falling, and with spontaneous recovery. Syncope must be carefully differentiated from other conditions that may cause a loss of consciousness or falling. Syncope can be classified into four categories: reflex mediated, cardiac, orthostatic, and cerebrovascular. ...

Diagnostic relevance of the carotid sinus massage during a head up tilt table test (HUTT).

BACKGROUND Syncope due to carotid sinus hypersensitivity (CSH) may be underdiagnosed due to a low level of diagnostic suspicion and fear of complications caused by massage of the carotid sinus. OBJECTIVE To investigate the relevance of carotid sinus massage in the diagnosis of non-convulsive faints and unexplained falls. METHODS Two-hundred and fifty-nine patients with a mean age of 50+/-24...
